Answer "Yes" if you have:

  • No prior diagnosis of colorectal cancer, adenomatous polyps, or inflammatory bowel disease

  • No personal diagnosis or family history of colorectal cancer or known genetic disorders such as Lynch syndrome or familial adenomatous polyposis

3. Are you due for colon cancer screening?

Yes

No

Answer "No" if you have completed any of the following screening tests:

  • Colonoscopy within the last 10 years

Or

Either of the following stool tests:

  • FIT/FOBT — in the last year

  • FIT-DNA — in the last 1-3 years

If all three answers are "Yes," you may be eligible for colon cancer screening.
